Job summary

Amedisys, part of the Optum family, is hiring a Hospice Liaison/Hospice Sales Rep to generate referrals and grow census by building relationships with physicians, hospitals, SNFs, and community providers. You will educate partners about hospice services, facilitate referrals, and collaborate with the interdisciplinary team with a compassionate, patient-first approach.

This role includes on-site visits, CRM documentation, and meeting admissions and payor-mix goals.

Qualifications

• 1+ years of experience in a healthcare related business development/outreach role.
• Reliable transportation, current driver's license, and liability insurance.
• Bachelors degree.

Responsibilities

• Educates health care professionals, patients, and families about home care services
• Develops and maintains relationships with key referral sources to generate leads
• Coordinates patient admissions with the team and completes referral to admission
• Conducts on-site visits to assess patient needs and identify barriers to care
• Keeps accurate records of patient interactions and referrals
• Achieves admissions, product, and payor-mix goals
• Meets short and long-term target account goals
• Documents the minimum expectation of sales calls daily in CRM
• Maintains records of targeted referral sources in territory in CRM
• Attends community events to promote Amedisys
• Responds to customer complaints in a timely manner
• Performs other duties as assigned
• Travel 50% or more within the assigned territory
