Job summary

Amedisys, part of the Optum family, seeks a Hospice Liaison/Hospice Sales Rep to drive referrals and census growth by building relationships with physicians, hospitals, and community providers.

The role educates partners about hospice services, facilitates referrals, and collaborates with the interdisciplinary team with a patient-first approach. Travel 50%+ within the territory is expected.

Qualifications

• 1+ year of experience in a healthcare related business development/outreach role
• Access to reliable transportation, current driver’s license, and liability insurance

Responsibilities

• Educates health care professionals, patients, and families about home care services
• Develops and maintains relationships with key referral sources to generate leads
• Coordinates patient admissions with the team and collects documentation for referral-to-admission
• Conducts on-site visits to assess patient needs and barriers to care
• Keeps accurate records of patient interactions and referrals in CRM
• Achieves admissions, product, and payor-mix goals
• Meets short and long-term target account goals
• Documents minimum daily sales calls in CRM with notes
• Attends community events to promote Amedisys
• Responds to customer complaints promptly
• Performs other duties as assigned
• Travel 50% or more within the assigned territory
