Job summary

Amedisys, part of the Optum family, is seeking a Hospice Liaison/Hospice Sales Rep to drive referrals and census growth by building relationships with physicians, hospitals, and community providers.

You will educate partners about hospice services, coordinate admissions, and collaborate with the interdisciplinary team to ensure smooth care transitions while maintaining patient-first compassion. Travel 50%+ within your territory is required.

Qualifications

• 1+ year of experience in a healthcare related business development/outreach role.
• Reliable transportation, current driver's license, and liability insurance.

Responsibilities

• Educates health care professionals, patients, and families about home care services.
• Develops and maintains relationships with key referral sources to generate leads.
• Coordinates patient admissions with the team, including obtaining documentation for referral to admission.
• Conducts on-site visits to assess patient needs and identify barriers to care.
• Keeps accurate records of patient interactions and referrals.
• Achieves admissions, product, and payor-mix goals.
• Meets short and long-term target account goals.
• Documents the minimum expectation of sales calls daily in CRM with notes.
• Maintains a complete, up-to-date record of targeted referral sources in territory in CRM.
• Attends community events to promote Amedisys.
• Responds to customer complaints in a timely manner.
• Performs other duties as assigned.
• Role requires travel 50% or more within territory.
