Job summary

Amedisys, part of the Optum family, is hiring a Hospice Liaison/Hospice Sales Rep to educate providers about hospice services, generate referrals, and drive census growth while coordinating with the care team.

The role requires building relationships with physicians, hospitals, nursing facilities, and community providers, traveling 50%+ within the territory, and maintaining accurate CRM records. The position offers a comprehensive benefits package and development opportunities.

Qualifications

• 1+ year of experience in a healthcare related business development/outreach role.
• Access to reliable transportation, valid driver's license, and liability insurance.

Responsibilities

• Educates health care professionals, patients, and families about home care services.
• Develops and maintains relationships with key referral sources to generate leads.
• Coordinates patient admissions with the team, including documentation for referral-to-admission.
• Conducts on-site visits to assess patient needs and barriers to care.
• Keeps accurate records of patient interactions and referrals in CRM.
• Achieves admissions, product, and payor-mix goals.
• Meets short and long-term target account goals.
• Documents the minimum expectation of sales calls daily in CRM.
• Maintains a complete, up-to-date record of targeted referral sources in territory in CRM.
• Attends community events to promote Amedisys.
• Responds to customer complaints in timely manner.
• Performs other duties as assigned.
• This role will travel 50% or more within the assigned territory.
