Job summary

VIA Health Partners in Stanly County, Albemarle, NC is seeking a dedicated Registered Nurse to join our hospice and palliative care team. The role focuses on coordinating and delivering patient/caregiver care, conducting nursing assessments, and developing plans of care to achieve goals while ensuring continuity of care.

Qualifications include a nursing diploma or associate degree, active compact RN license, CPR certification, and computer proficiency.

Qualifications

Diploma or Associate Degree in Nursing is required; Bachelor's Degree preferred.
Current RN license, active and unrestricted in a multistate (NLC) compact.
CPR certification required; CHPN preferred.
Computer proficiency is required.

Responsibilities

Coordinate and deliver patient/caregiver care.
Conduct nursing assessments to manage patient/caregiver needs.
Develop, implement, and update the plan of care to ensure goals are achieved.
Educate patients and families; provide infection control and safety guidance.
Communicate with IDG, patients, caregivers, and other health professionals to ensure continuity of care.
Maintain accurate and timely clinical documentation.
Demonstrate time management and respond to crises as needed.
