Job summary

VIA Health Partners Hospice in Charlotte, NC seeks a Registered Nurse to join our end-of-life care team at Hospice House Southminster. You will coordinate patient/caregiver care, perform nursing assessments, and implement care plans to achieve goals with an emphasis on comfort and family support.

You’ll educate patients and families, collaborate with an interdisciplinary group, maintain infection control, and uphold professional standards in a compassionate, community-based setting.

Qualifications

• A Diploma or associate degree in nursing is required; Bachelor's Degree preferred.
• A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
• Current CPR required; CHPN preferred.
• Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions. Experience in direct care of patients with life limiting illnesses required.
• Computer proficiency is required.

Responsibilities

• Assumes responsibility for the coordination and delivery of patient/caregiver care.
• Conducts nursing assessments to assist in the overall management of patient/caregiver needs.
• Develops, implements, evaluates, and updates the plan of care to ensure that care is uninterrupted and that goals are achieved.
• Facilitates patient and caregiver independence to the extent possible.
• Communicates effectively with the IDG, patients, caregivers, and other health professionals to ensure continuity of care.
• Maintains open lines of communication with supervisor and reports complex issues.
