Qualifications

• RN with active multistate license required.
• Diploma or ADN required; Bachelor’s preferred.
• Current CPR; CHPN preferred.

Responsibilities

• Coordinate and deliver patient/caregiver care in hospice setting.
• Conduct nursing assessments to manage patient needs.
• Develop and update plans of care to ensure continuous, goal-aligned care.
• Educate patients and families; engage in caregiver education.
• Maintain infection control practices and universal precautions.
