Job summary

VIA Health Partners in Davidson, NC is seeking a Registered Nurse to coordinate and deliver patient and caregiver care in an end-of-life setting. You will perform nursing assessments, develop and update the plan of care, and educate patients and families.

You will work with the IDG and other health professionals to ensure continuity of care, while adhering to infection control and documentation standards. Requires RN licensure, CPR, and preferred CHPN certification; Diploma or Associate Degree

Qualifications

• Diploma or Associate Degree in Nursing required
• RN license active and unrestricted
• CPR certification required; CHPN preferred
• Computer proficiency required

Responsibilities

• Coordinate and deliver patient/caregiver care
• Conduct nursing assessments and update care plans
• Educate patients and families and facilitate questions
