Job summary

Choice Health At Home is seeking a Hospice Aide for our Tyler Hospice Branch in Tyler, TX. The role involves providing direct patient care under RN supervision, assisting with transfers and ambulation, and documenting care to ensure continuity for families.

Qualifications

• Current CNA/HHA and LTC certifications required

• CPR Certification required

Responsibilities

• Provide direct patient care to patients under direction of the RN and according to the Care Plan
• Assist patients with safe transfers and ambulation per agency policy
• Report changes and document pertinent information to ensure continuity of care
• Maintain infection control principles to provide a clean, safe environment
• Promote respectful communication with patient/family and staff
• Assist with nutrition and fluid intake as needed
• Support efficient operation and time management within the agency
