Job summary

Choice Health At Home is seeking a Hospice Field Registered Nurse (RN) to deliver patient-centered care in home settings across Brazoria County. You will coordinate comprehensive care plans addressing physical, emotional, and spiritual needs while partnering with the IDG to ensure comfort and dignity.

Responsibilities include initial and ongoing assessments, determining care levels, developing and documenting the plan of care, educating patients and families, providing on-call coverage, and

Qualifications

• Current RN license is required.
• CPR certification is required.
• Valid driver’s license and auto insurance are required.
• Hospice RN experience is preferred (2+ years).
• Experience with HomeCare HomeBase is preferred.

Responsibilities

• Initial and ongoing comprehensive assessments of patient needs across physical, functional, psychosocial factors.
• Determine admission and re-certification criteria for hospice care to establish levels of care.
• Develop and document the plan of care; revise as needed for quality and continuity.
• Educate patient and family about disease process, end-of-life care, and self-care needs.
• Provide on-call coverage and guide hospice aides and LPNs/LVNs to ensure continuity of service.
• Inform physicians of changes in patient needs and outcomes; document progress.
