Job summary

Choice Hospice, Inc. is seeking a dedicated Hospice Field Registered Nurse (RN) to provide patient-centered nursing care in Austin, TX and surrounding areas.

The role emphasizes managing comprehensive care plans and collaborating with an interdisciplinary team to support patients and families. The ideal candidate holds an active RN license, CPR certification, and a valid driver’s license, with preference for 2+ years of hospice RN experience and familiarity with HomeCare HomeBase.

Qualifications

• Current RN License is required.
• CPR Certification is required.
• Valid Driver’s License required.
• Valid Auto Insurance required.
• HomeCare HomeBase experience preferred.
• Hospice RN experience of 2+ years preferred.

Responsibilities

• Perform initial and ongoing assessments of patients' physical, functional, psychosocial, and environmental needs.
• Apply admission and recertification criteria to determine hospice eligibility and level of care.
• Develop and document the care plan to ensure quality and continuity of care.
• Educate patients and families about disease process, end-of-life care, nutrition, and self-care; train other staff as needed.
• Determine service scope and frequency based on acuity; assess caregiver ability.
• Give clinical directions to Hospice Aide and LPN/LVN for continuity of service.
• Prepare progress notes showing progress toward goals and maintain documentation.
• Inform physicians and staff of changes in patient needs and responses to care.
• Perform on-call responsibilities as assigned.
