Job summary

Choice Health at Home is seeking a Hospice Field Registered Nurse (RN) to provide compassionate, patient-centered care in patients' homes across Corpus Christi, TX. The role focuses on comprehensive nursing care plans, symptom management, and coordination with a multidisciplinary team to ensure comfort and dignity at end of life.

You will perform assessments, develop and update plans of care, educate patients and families, and support the care team with on-call coverage.

Qualifications

• Current RN license in good standing.
• CPR certification is required.
• Valid driver’s license and auto insurance.
• Hospice RN experience preferred.
• Experience with HomeCare HomeBase is a plus.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs.
• Develop, implement, and update the plan of care to ensure quality.
• Educate patients and families about disease processes and at-home care.
• Coordinate with physicians, social workers, and chaplains to support holistic care.
• Provide on-call coverage and ensure timely documentation of patient progress.
• Communicate changes in patient needs to the care team.
