Choice Hospice, Inc. is seeking a Hospice Field RN to provide compassionate, in-home and community-based nursing care for patients facing life-limiting illnesses.

The role emphasizes coordinated care plans, comfort, dignity, and support for families, working with physicians, social workers, and chaplains to tailor care to each patient’s needs. The RN will conduct initial and ongoing assessments, apply admission criteria, develop and document plans of care, educate families on end-of-life care,

Qualifications

• Current RN License.
• CPR Certification.
• Valid Driver’s License.
• Valid Auto Insurance.
• At least 2 years of Hospice RN experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform initial and ongoing comprehensive assessments of the impact of terminal disease on patient needs.
• Apply admission and re-certification criteria for hospice care to establish levels of care.
• Develop and document the plan of care and revise as necessary.
• Educate patient and family about disease process, end-of-life care, nutrition, and self-care techniques.
• Provide training to other staff as needed.
• Determine scope and frequency of services based on acuity and family needs.
• Provide clinical directions to Hospice Aide and LPN/LVN to ensure quality care.
• Prepare progress notes showing progress toward goals and maintain documentation.
• Inform physician and team of changes in patient needs and intervention outcomes.
• Perform on-call responsibilities and provide on-call service.
