Job summary

The Joliet Area Community Hospice is seeking an RN Field Case Manager to deliver skilled nursing assessments and hospice care to patients and families in home and community settings.

This role requires a current RN license, 2 years nursing experience (1 year in public health/home care/hospice preferred), and a valid driver's license. Monday–Friday with on-call hours across Southern Cook County.

Qualifications

• Current RN license.
• Graduate of accredited nursing program.
• Minimum of 2 years of nursing experience, with at least 1 year in public health, home care, or hospice preferred.
• Valid driver’s license and insured, reliable vehicle.

Responsibilities

• Deliver skilled nursing care and hospice services in home and community settings.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications and treatments while monitoring patient symptoms and outcomes.
• Educate patients and caregivers on care, self-management, and disease progression.
• Collaborate with the interdisciplinary team to ensure coordinated, high-quality care.
• Maintain accurate documentation and participate in on-call coverage as needed.
• Coverage will include Southern Cook County (Palos Park, Crestwood, Hickory Hills & Surrounding Areas).
• Monday–Friday schedule
• Minimum of 20 on-call hours per month (may increase).
