Job summary

MJHS Hospice and Palliative Care is seeking a Field Case Manager to visit patients in community and facility settings, perform physical and psychosocial assessments, and document visits electronically. You will develop and implement nursing care plans and make recommendations tailored to patient needs.

The role requires NYS RN license and NYS Driver’s license, with a schedule that includes one weekend per month and backup On Call 2–3 times monthly.

Qualifications

• Associates/Diploma in Nursing from an accredited school.
• BSN preferred.
• 2+ years’ experience in med/surg, community health, and/or oncology nursing setting preferred.
• Must hold NYS RN license.
• Must have NYS Driver’s license.

Responsibilities

• Visit MJHS Hospice patients to perform physical and psychosocial assessments.
• Electronically document visit occurrences.
• Develop and implement nursing plans of care.
• Make recommendations for care based on patients' needs.
• Perform field-based visits in community and facility settings.
• Work schedule includes one weekend per month and backup On Call 2–3 times per month; car available for On Call shifts.
