MJHS Hospice and Palliative Care seeks a field-based Hospice Field Case Manager to perform comprehensive nursing assessments during patient visits in community and facility settings. You will develop and implement nursing care plans and provide recommendations to address patient needs.
The role includes one weekend per month and backup on-call 2–3 times per month, with car and escort support for on-call shifts.
Formación
- Nursing diploma/associates required; BSN preferred.
- 2+ years’ experience in med/surg, community health, and/or oncology nursing setting preferred.
- Valid NYS RN license required; NYS Driver’s license required.
Responsabilidades
- Visit MJHS Hospice and Palliative Care patients to perform physical and psychosocial assessments and document visit occurrences.
- Develop and implement nursing plans of care and make recommendations based on patients’ needs.
- Work in both community and facility settings; schedule includes one weekend per month and backup On Call 2–3 times per month; car and escort service provided for On Call shifts.
