Job summary

The Hospice RN Case Manager at JOL Healthcare, Inc. coordinates and delivers high-quality hospice care to patients in homes and facility settings. You will develop individualized plans, manage visits, and collaborate with an interdisciplinary team while ensuring compliance with hospice standards.

You will document all care in the EMR, educate families on disease progression, and serve as the primary contact for patients and families. Local travel is required with compassion and professionalism.

Qualifications

• Graduate of an accredited nursing program and active RN license.
• Hospice/palliative care certification preferred.
• Minimum 2 years nursing experience; 1 year in hospice preferred.
• Strong assessment, critical thinking, and communication skills.
• Ability to travel locally to patient homes.

Responsibilities

• Coordinate and provide skilled nursing care and symptom management.
• Develop and implement individualized care plans.
• Educate patients and families on disease progression and care expectations.
• Document all patient interactions in the EMR system.
• Collaborate with physicians, social workers, chaplains and other team members.
• Participate in interdisciplinary team meetings and care planning.
• Ensure compliance with hospice policies and regulatory requirements.
