Job summary

Trinity Health At Home is seeking a Home Care Hospice Educator to lead education initiatives for the home care team. You will assess needs, develop and implement education programs, and evaluate outcomes while ensuring compliance with local, state, and federal regulations.

Qualifications

• Graduate of an approved Nursing education program, bachelor’s in nursing is preferred.
• Ten years of nursing practice, including five in Hospice education or leadership
• Preferred CPR Instructor Certification.
• Must be proficient in the use of Microsoft applications for creation of instructional documents, course curriculums, orientation materials and related visual aides
• Must possess accurate and clear oral & written communication skills

Responsibilities

• Assess, develop, implement and evaluate education for home care with emphasis on regulatory compliance.
• Collaborate with home care managers to orient, develop and integrate new colleagues and ongoing staff development.
• Serve as informatics liaison and educate on technologies (Epic EHR, learning management system, etc.).
• Coordinate student learning activities with schools, colleges and universities.
