Job summary

Altus Hospice in San Antonio seeks a self-driven Community Liaison to grow census by building trust-based relationships with referral sources, physicians, and healthcare partners. You will be the face of Altus Hospice, promoting our specialized services across the community while championing compassionate, patient-centered care.

The role focuses on outreach, education, and establishing a strong referral network to ensure access to hospice services.

Qualifications

• Bachelor’s degree in related field preferred.
• Minimum two years of sales/marketing experience in healthcare.
• Hospice/palliative marketing experience strongly preferred.
• Skilled at establishing/maintaining relationships with key staff, marketing contracts, and clinical staff.
• Proven ability to generate and maintain relationships with physicians, healthcare professionals, and community agencies.
• Prior clinical experience is a plus.
• Willing to drive with reliable transportation and valid license/auto insurance.

Responsibilities

• Establish and nurture relationships with key decision-makers, including physicians and healthcare partners.
• Serve as a passionate advocate for our hospice services through outreach.
• Grow presence by positioning hospice as provider of choice across the service area.
• Drive referral growth through regular visits with referring physicians, hospital discharge planners, SNFs, social workers, and other professionals.
• Support patients and families by leading initial conversations about hospice care and explaining services with empathy.
