Altus Hospice Care is seeking a self-driven Community Liaison to join our San Antonio team. You’ll be the face of our hospice, building trust-based relationships with referral sources, physicians, and healthcare partners to grow census and awareness in the Med Center and NE San Antonio.

The role emphasizes outreach, educating families about hospice services with empathy, coordinating referrals, and supporting patients through transitions.

Qualifications

• Bachelor’s degree preferred; substitution may be considered.
• Minimum two years of healthcare sales/marketing experience.
• Experience establishing and maintaining relationships with staff, marketing contracts, and clinical teams.
• Proven ability to build relationships with physicians and healthcare professionals.
• Must be willing to drive with reliable transportation and a valid license.

Responsibilities

• Establish and nurture relationships with key decision-makers to strengthen collaboration within the medical community.
• Serve as a hospice advocate through regular outreach reflecting commitment to compassionate, high-quality care.
• Advance census growth by promoting hospice services across the service area.
• Drive referral growth through visits with referring physicians, discharge planners, SNFs, social workers, and other professionals.
• Lead initial conversations about hospice care, explain services with empathy, and obtain consent.
