Job summary

Residential Home Health and Hospice in Ann Arbor, MI is seeking a Certified Nursing Assistant to join our hospice team delivering compassionate, in-home care. You will work with an interdisciplinary group to support comfort, dignity, and quality of life for patients in their homes.

This role requires an active CNA certificate, a high school diploma, and at least one year of healthcare experience; a valid driver's license is needed for roughly 20% daily travel.

Qualifications

• Active CNA certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience; hospice experience preferred
• Valid driver’s license and ability to drive approx. 20% of workday

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting
• Provide hands-on care to patients and assist with ADLs
• Support and guide patients and caregivers by reinforcing the care plan
• Respond promptly to patient needs with empathy and documenting care in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
