Job summary

Residential Home Health and Hospice in Ann Arbor, MI is seeking a Certified Nursing Assistant to join our hospice team delivering compassionate, in-home care. You will work with an interdisciplinary group to support comfort, dignity, and quality of life for patients in their homes.

This role requires an active CNA certificate, a high school diploma, and at least one year of healthcare experience; a valid driver's license is needed for roughly 20% daily travel.

What You'll Do:

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care to patients, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support comfort and quality of life
• Support and guide patients and caregivers by reinforcing the care plan, offering emotional support, and helping them understand what to expect during the hospice journey
• Respond promptly to patient needs with empathy and attentiveness and accurately document care provided in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
