Residential Home Health and Hospice in Springfield, IL is seeking a dedicated Hospice Certified Nursing Assistant (CNA) to provide patient-centered care in home settings. Qualifikationen

• Deliver compassionate, patient-centered care in a hospice setting
• Assist with activities of daily living (ADLs) and observe changes in condition
• Support patient and caregiver by reinforcing the care plan and providing emotional support
• Document care provided in the EMR and communicate with the interdisciplinary team
• Work independently while collaborating with the team to ensure comfort and quality of life
• Travel to patient homes within the assigned territory
