Job summary

Residential Home Health and Hospice in Ann Arbor, MI is seeking a Hospice Certified Nurse Assistant (CNA) to provide compassionate home-based care. You will work in a hospice setting, perform ADLs, document care in the EMR, and collaborate with an interdisciplinary team to ensure comfort and quality of life for patients.

The role requires an active CNA in the state, a valid driver's license, and the ability to travel to patient homes.

Qualifications

• Active CNA certificate in the state you work.
• High School Diploma or equivalent.
• Minimum one year healthcare experience; hospice experience preferred.
• Valid driver's license and ability to drive ~20% of workday.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting.
• Provide hands-on care to patients and assist with ADLs.
• Reinforce care plans, offer emotional support to patients and caregivers.
• Respond to patient needs promptly and document care in the EMR.
• Work independently, but not alone.
• Travel to patient homes within assigned territory.
