Job summary

Moments Hospice is seeking a Certified Nursing Assistant (CNA) / Hospice Aide to join our field-based team in Minnesota. Earn $18–$23 per hour while providing compassionate personal care to patients at home, assisted living, or care facilities.

Work Monday–Friday, 8:30 AM–5:00 PM, with structured training and supportive colleagues. You will collaborate with an RN Case Manager, maintain EMR documentation, and help families find comfort and peace in the final chapter of life.

Qualifications

• Active CNA certification required.
• Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice a plus.
• Current CPR certification preferred.
• Ability to work in an electronic medical record system.
• Strong communication skills, including written and verbal.
• Excellent customer service, problem-solving, and time management skills.
• Compassion, patience, and the ability to handle emotional situations with care.
• Ability to lift 50 lbs.
• Valid driver’s license and current auto insurance (required).

Responsibilities

• Provide personal care with activities of daily living, including bathing, dressing, grooming, toileting, and skin care.
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation.
• Assist with dining and nutrition.
• Review and follow each patient’s plan of care under the direction of the RN Case Manager.
• Provide comfort and companionship to patients and support to their families.
• Maintain electronic medical records.
