Job summary

Moments Hospice is seeking a compassionate Certified Nursing Assistant (CNA) to provide hands-on personal care to patients in homes, assisted living, or care facilities. You will support daily living activities, monitor comfort, and document care under an RN Case Manager as part of the interdisciplinary team.

Qualifications

Active CNA certification required.
Experience in home care, assisted living, or hospice a plus.
Current CPR certification preferred.
Ability to work in an electronic medical records system.

Responsibilities

Provide personal care (bathing, dressing, grooming, toileting, skin care).
Assist with lifting, transfers, positioning, ROM, and ambulation.
Assist with dining and nutrition.
Follow patient care plans under RN direction.
Provide comfort and companionship; support families.
Maintain electronic medical records.
