Moments Hospice is seeking a Certified Nursing Assistant (CNA) / Hospice Aide in Wisconsin to join our field-based team. Pay ranges from $18 to $23 per hour, with full-time hours and a Monday–Friday 8:30 AM–5:00 PM schedule.

You will provide hands-on personal care in patients' homes, assist with daily living activities, and support families as part of an interdisciplinary care team. We offer a fleet car program, company cell phone, comprehensive insurance options, paid time off, and a 401(k)

Qualificações

• Active CNA certification required.
• Healthcare caregiving experience in home care, assisted living, nursing facility, hospital, or hospice a plus.
• Current CPR certification preferred.
• Ability to work in an electronic medical record system.
• Strong communication skills, written and verbal.
• Ability to lift 50 lbs. and understand body mechanics.

Responsabilidades

• Provide personal care with activities of daily living (bathing, dressing, grooming, toileting, skin care).
• Assist with lifting, transferring, positioning, ROM, exercise, and ambulation.
• Assist with dining and nutrition.
• Follow patient care plans under RN direction.
• Provide comfort and companionship to patients and support to families.
• Maintain electronic medical records.
