Job summary
Moments Hospice is seeking a dedicated Certified Nursing Assistant (CNA) / Hospice Aide to join our field-based team. You will provide hands-on personal care, help with activities of daily living, and support patients in homes or care facilities under the RN Case Manager's guidance.

Qualifications
- Active CNA certification is required.
- Healthcare caregiving experience is a plus.
- CPR certification is preferred.
- Ability to operate in an electronic medical record system.
- Excellent communication and time-management skills.
- Compassion, patience, and professionalism in handling emotional situations.
- Ability to lift up to 50 lbs.
Responsibilities
- Provide personal care including bathing, dressing, grooming, toileting, and skin care.
- Assist with lifting, transferring, positioning, range of motion, and mobility.
- Assist with dining and nutrition.
- Follow patient care plans under RN supervision.
- Provide comfort and companionship to patients and support to families.
- Maintain electronic medical records.
