Zusammenfassung

Moments Hospice in Wisconsin is seeking a Certified Nursing Assistant (CNA) / Hospice Aide to provide hands-on personal care for patients at home or in care facilities. The role is full-time, field-based, with a Monday–Friday schedule and pay of $18–$23 per hour.

You will work closely with an RN Case Manager on an interdisciplinary team, follow care plans, document in electronic medical records, and support families with compassionate care.

Qualifikationen

• Active CNA certification is required.
• Healthcare caregiving experience in home care, assisted living/memory care, skilled nursing, hospital, or hospice a plus.
• Current CPR certification is preferred.
• Ability to work in an electronic medical record system.
• Strong communication, time management, and customer service skills.
• Compassionate, patient, and able to handle emotional situations with care.
• Valid driver's license and current auto insurance are required.

Aufgaben

• Provide personal care with activities of daily living, including bathing, dressing, grooming, toileting, and skin care.
• Assist with lifting, transferring, positioning, range of motion, exercise, and ambulation.
• Assist with dining and nutrition.
• Review and follow each patient’s plan of care under the direction of the RN Case Manager.
• Provide comfort and companionship to patients and support to their families.
• Maintain electronic medical records.
