Job summary

Responsibilities include providing personal care, meals, and observation of patient condition, with reporting to the Case Manager. Hours Monday–Friday, no weekends or holidays in this role.

Qualifications

• Certified by the state as a nurse aide.
• Listed on the state aide registry with a clear record.
• 18 years of age or older per state law.
• Valid driver’s license with insured vehicle.

Responsibilities

• Giving personal care including baths and grooming.
• Assisting in dressing and undressing patients.
• Planning and preparing nutritious meals, including shopping.
• Assisting in feeding the patient.
• Monitoring skin condition to prevent breakdown.
• Reporting patient status and changes to the Case Manager.

Monday-Friday 8:30AM-5PM - NO WEEKENDS - NO HOLIDAYS

What You�ll Do

• Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linens
• Assisting in dressing and undressing patients
• Planning and preparing nutritious meals, including shopping
• Assisting in feeding the patient
• Providing proper care and observation of patient�s skin to prevent breakdown of tissue
• Reporting on patient’s condition and significant changes to the Case Manager.
