Job summary

JourneyCare Hospice is seeking Hospice CNAs for our growing team in Morton Grove, IL. This full-time role offers competitive salaries, comprehensive benefits, and a clear path for career growth.

You will provide patient care, assist with daily living activities, and report changes to the Case Manager. Highlights include generous time off, 401(k) with match, tuition reimbursement, and a structured schedule: Monday–Friday, 8:30 AM–5 PM, with no weekends or holidays.

Qualifications

• Certified by the state as a nurse aide.
• List on the state aide registry with a clear record.
• 18 years of age or older (state law dependent).
• Valid driver’s license; insured vehicle as required by state.

Responsibilities

• Provide personal care (baths, back rubs, oral hygiene, shampoos, linen changes).
• Assist in dressing and undressing patients.
• Plan and prepare nutritious meals; assist with shopping.
• Assist in feeding patients.
• Monitor and report skin condition to prevent tissue breakdown.
• Report changes to the Case Manager promptly.

What You’ll Do:

• Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linens
• Assisting in dressing and undressing patients
• Planning and preparing nutritious meals, including shopping
• Assisting in feeding the patient
• Providing proper care and observation of patient’s skin to prevent breakdown of tissue
• Reporting on patient’s condition and significant changes to the Case Manager.
