Job summary

Enhabit Home Health & Hospice is seeking an experienced RN to lead hospice care coordination, ensuring delivery per standards and regulations. You will review patient data at start of care and during recertifications, and foster strong communication among doctors, referrals, patients, and caregivers.

This role emphasizes scheduling, team collaboration, and adherence to federal, state, and local guidelines, with a preference for Homecare Homebase experience and bilingual communication.

Qualifications

• Graduate of an approved school of professional nursing.
• Valid clinical nursing license per state requirements.
• Experience with hospice or home health preferred.
• Knowledge of federal, state and local home care laws.
• CPR certification maintained.
• Valid driver license and auto liability insurance.
• Proficient with Microsoft Office tools.
• Clear communication with patients and caregivers in English; bilingual in English/Spanish preferred.

Responsibilities

• Coordinate hospice services delivered to the patient per standards and procedures.
• Review and approve patient information during start of care, recertification, resumption of care, or evaluation visits.
• Facilitate relationships among physicians, referral sources, patients, caregivers and staff.
• Schedule patients to field clinicians and maintain care coordination and continuity.
