Job summary

Ohio Living is seeking an experienced Hospice Operations Leader to oversee clinical team coordination, care planning, and IDG meetings in our Ohio hospice programs.

The role requires an Ohio RN license (BSN preferred) with 3 years in clinical operations supervision and hospice exposure. Proficiency in EMR and MS Office is essential, with on-call duties and potential Alternate Administrator responsibilities.

Qualifications

  • Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) required.
  • Three years experience in clinical operations supervision required.
  • One year experience in a hospice setting required.
  • Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet required.
  • Proficiency with electronic medical records (EMR) preferred.

Responsibilities

  • Facilitates clinical team with coordination and oversight of patient/family care services and comprehensive care plans.
  • Supervises implementation of the hospice plan of care and coordinates IDG resources to reach outcomes.
  • Oversees hospice IDG meetings and documentation.
  • Interprets operational indicators to detect census changes affecting staffing, revenue, and expenses.
  • Audits and maintains medical records and clinical documentation per standards and regulations.
  • Participates in on-call rotation and may assume Alternate Administrator duties when needed.
