Job summary

Ohio Living is seeking a Hospice Clinical Supervisor to lead the clinical team in delivering compassionate end-of-life care. You will coordinate care plans, supervise IDG activities, and ensure compliance with state and federal standards while optimizing resource use and data quality.

The role requires an Ohio RN license with at least three years in clinical operations supervision and one year in hospice or Medicare-certified home health settings.

Qualifications

• Current unencumbered license for the state of Ohio to practice as a Registered Nurse (RN) required.
• Three years experience in clinical operations supervision.
• One year experience in a hospice setting.
• One year experience in a Medicare-certified home health agency required.
• Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet.
• Proficiency with electronic medical records (EMR) preferred.
• Must be able to read, write, speak, and understand English.
• Must possess a valid driver's license and acceptable driving record.

Responsibilities

• Coordinate and oversee direct and indirect patient/family care services with comprehensive care plans.
• Oversee the interdisciplinary group (IDG) reviews and revise the plan of care.
• Organize IDG meetings at least every 15 days.
• Interpret operational indicators to detect census changes and staffing needs.
• Review diagnosis coding and plan of care orders per Medicare regulations.
• Ensure equipment and monitoring meet quality and compliance standards.
• Collaborate with marketing to manage referrals and patient needs.
• Manage local EMR component and train staff on proper use.
• Evaluate the plan of care effectiveness and staff performance.
• Evaluate workflow, administrative, and departmental processes.
• Monitor hospice revenue and collections and ensure accurate documentation.
• Conduct supervised patient visits and participate in on-call rotation.
• May assume Alternate Administrator duties during absences.
