As Clinical Services Manager, you will oversee care coordination, staff development, and compliance, with focus on patient-centered care and outcomes. Wisconsin licensure required, BSN preferred, and leadership experience expected; salary between 80,000 and 90,000

Qualifications

• Active RN license in Wisconsin.
• BSN preferred.
• Minimum of 2 years leadership or supervisory experience.
• Home health or hospice experience preferred.
• Strong clinical judgment, communication, and organizational skills.
• Ability to lead and collaborate within an Interdisciplinary team.
• Understanding of hospice philosophy and end-of-life care.
• Knowledge of pain and symptom management.
• Valid driver’s license with a reliable, insured vehicle.

Responsibilities

• Managing patient referrals, reviewing cases, and assigning clinical staff.
• Overseeing development, implementation, and updates of individualized care plans.
• Collaborating with physicians and Medical Director regarding patient needs.
• Supporting clinicians with goal setting, care planning, and clinical decision-making.
• Triaging and addressing concerns from patients, families, facilities, and providers.
• Approving and managing clinical workflows for home health and hospice services.
