Job summary

Affinity Hospice Holdings, LLC is seeking a Clinical Manager to coordinate and direct hospice clinical services. The role includes guiding clinicians, evaluating cases, and ensuring service quality across home visits and team conferences.

The position requires state RN licensure, a BSN preferred, and at least two years of management experience in hospice or home care. A valid driver’s license and CPR certification are essential; this is a full-time on-site role with a comprehensive benefits

Qualifications

• Registered Nurse licensed in the state.
• BSN preferred; NLN-accredited program.
• Minimum of two years management or supervisory experience in a hospice or home care setting.
• Proven ability to work effectively within an interdisciplinary team.
• Active and unencumbered RN license in the state of employment.
• Valid driver’s license with insured vehicle.
• Current CPR certification.

Responsibilities

• Review case referrals and assess patient needs, including home visits.
• Assign hospice associates and collaborate with Medical Director to determine eligibility for services.
• Oversee and evaluate patient cases through home visits, conferences, and record reviews.
• Provide guidance to clinicians to improve performance and service delivery.
• Review patients' diagnoses, prognoses, medications, procedures, and clinical courses.
• Assist clinicians in developing effective care plans and goals.
• Lead case conferences to ensure care coordination and collaboration.
• Conduct quarterly record reviews and provide recommendations to leadership.
• Participate in hiring, screening, and orientation for new staff; mentor teammates.
• Plan, implement, and evaluate in-service and continuing education programs for staff.
• Support policy and procedure development; ensure compliance with standards.
• Assess community health needs and contribute to program development.
• Engage in public relations and community activities to promote hospice care.
• Participate in quality assessment and performance improvement initiatives.
• Perform other duties as delegated by the Executive Director.
• Familiarity with accounting practices and financial reporting; detect discrepancies.
