Job summary

The Pennant Group, Inc. seeks a Clinical Services Manager (RN) to provide leadership across our home health and hospice services in Wisconsin. This full-time role requires an active Wisconsin RN license and strong clinical and supervisory skills.

You will oversee care coordination, staff development, and performance improvement initiatives, lead IDG meetings, perform chart reviews, and ensure regulatory compliance across programs.

Wage: $80,000-$90,000/year

As a Clinical Services Manager, you will oversee care coordination, guide clinical staff, and ensure exceptional service delivery across home health and hospice programs.

What You’ll Do

As a Clinical Services Manager, you will provide leadership and oversight to ensure quality care and efficient operations.

• Managing patient referrals, reviewing cases, and assigning appropriate clinical staff
• Overseeing the development, implementation, and updates of individualized plans of care
• Collaborating with physicians and the Medical Director regarding patient eligibility and care needs
• Supporting clinicians with goal setting, care planning, and clinical decision-making
• Triaging and addressing concerns from patients, families, facilities, and providers
• Approving and managing clinical workflows for home health and hospice services
• Conducting home visits, record reviews, and performance evaluations to ensure quality care
• Leading and participating in interdisciplinary team (IDG) meetings and case conferences
• Performing chart audits, supervisory visits, and quality improvement initiatives (QAPI)
• Supporting hiring, onboarding, and development of clinical staff
• Reviewing DME and pharmacy orders for accuracy and appropriateness
• Monitoring outcomes and educating staff to support performance improvement
• Managing compliance processes such as Face-to-Face (F2F) visits
• Participating in leadership on-call rotation and completing visits as needed

What We’re Looking For

• Current Registered Nurse (RN) license in the State of Wisconsin
• BSN preferred
• Home health or hospice experience preferred
• Minimum of 2 years of leadership or supervisory experience preferredStrong clinical judgment, communication, and organizational skills
• Ability to lead and collaborate within an interdisciplinary team
• Understanding of hospice philosophy and end-of-life care
• Knowledge of pain and symptom management
• Ability to manage multiple priorities in a fast-paced environment
• Valid driver’s license with a reliable, insured vehicle
