Job summary

Medi Home Hospice in State College, PA is seeking a Clinical Manager to lead hospice clinical operations and ensure compliant, compassionate patient care.

Responsibilities include supervising clinical services, maintaining regulatory standards, and guiding care teams to uphold dignity and comfort for patients and families. RN license in PA and hospice supervision experience preferred.

Qualifications

• Associate’s degree in Nursing from an accredited program.
• Current PA RN license is required.
• Minimum 2 years of nursing experience (preferred).
• At least 1 year of hospice supervisory experience (required).
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide day-to-day clinical oversight and supervision of hospice patient care services.
• Ensure care meets federal and state regulations, accrediting body standards, and company policies.
• Review and approve Start of Care, Re-certifications, and Discharges.
• Audit patient records for completeness, accuracy, and compliance.
• Liaise between interdisciplinary teams, patients and families to ensure quality outcomes.
