Job summary

Hospice Buffalo is seeking a Clinical Outreach Specialist to educate communities about hospice and palliative care services, build partnerships with clinical providers, and identify patient needs to connect them with program offerings.

This full-time role requires RN, a valid driver’s license, and the ability to travel; the schedule includes evenings and weekends for events and outreach. Comprehensive benefits are provided.

Qualifications

• RN requirement.
• 1 year of experience with health care or clinical marketing/sales preferred.
• Experience in non-profit workplace helpful.

Responsibilities

• Provide education about HPCB mission, services/programs, and benefits in all community and clinical settings.
• Maintain partnerships within the clinical community and the community at large to provide education and assist with access to services.
• Identify needs of individuals and match them with the appropriate services offered through HPCB programs.
• Travel to urban, suburban, and rural areas as needed.
• Ability to work a flexible schedule, including nights and weekends to assist with networking events, education, family meetings, health fairs and other activities.
