Job summary

Hospice & Palliative Care Buffalo is seeking a Clinical Field Educator to provide ongoing education to clinicians from orientation through their two-year anniversary, ensuring a solid foundation in hospice and palliative care.

The role requires an NY-licensed RN with at least two years of hospice/palliative care experience, strong time management, and excellent communication to coordinate education and field supervision across departments.

Qualifications

• Graduate of an accredited school of nursing with NY RN licensure.
• Experience in hospice and/or palliative care is preferred.
• Must have valid driver's license and vehicle for on-call visits.

Responsibilities

• Provide ongoing education to clinicians from orientation through their 2nd anniversary.
• Coordinate and document progression and competence during visits.
• Support education remediation and annual reviews with Clinical Education team.
• Assist with record reviews for clinical accuracy and compliance.
• Cover weekend and off-shift visits as needed to support teams.
