Job summary

Heart N Home Hospice, part of the LHC Group within the Optum family, seeks a Clinical Director to assist the Executive Director in clinical oversight of hospice services. You will supervise patient care, ensure regulatory compliance, and motivate staff while maintaining agency philosophy and profitability.

You will lead clinical operations, collaborate with physicians, and drive quality and cost-effective care in a post-acute setting.

Qualifications

• Current and unrestricted RN licensure in the state of practice.
• Current CPR certification required.
• Current Driver's License and vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Ensure compliance with all hospice regulations, laws, policies and procedures, including Medicare/Medicaid hospice requirements.
• Oversee that the hospice agency employs only qualified hospice personnel.
• Present on-site during business hours or available by telephone when off-site on agency business.
• Direct day-to-day clinical operations, training, regulatory compliance and staff development.
• Oversee all patient care activities to ensure compliance with nursing and medical standards and regulatory guidelines.
• Promote hospice education to referral sources and the community.
• Collaborate with hospice physicians and community physicians to drive clinical excellence.
• Ensure patient care is delivered according to the plan of care and physician orders.
• Provide clinical oversight and supervision per licensure and state guidelines.
• Participate in Governing Body activities and support quality oversight.
• Support hospice billing processes to meet regulatory requirements.
• Ensure adequate staffing through recruitment and retention activities.
• Complete staff evaluations in a timely manner.
• Identify education needs and coordinate clinical training for staff.
• Review monthly financials and cost reports with leadership to maintain cost-effective care.
• Contribute to QA/QI programs with objective data to improve patient outcomes.
