Job summary

Amedisys, part of the Optum family, seeks an RN Clinical Manager Hospice to lead home health and hospice patient care operations and ensure high-quality, compliant services. You will supervise nurses and aides, coordinate referrals, admissions, scheduling, and documentation, while driving quality outcomes and fiscal stewardship.

In this role, you will collaborate with the director of staffing, participate in on-call duties, and train staff to uphold care standards and value-based care principles.

Qualifications

• RN license in the applicable state
• CPR certification
• 1+ years of clinical RN experience in home health or hospice
• 1+ years of RN management experience

Responsibilities

• Supports, participates and works cooperatively with the director of staffing.
• Responsible for overall quality of patient care and clinical performance of nurses and aides.
• Organizes clinical operations for the care center.
• Reviews requests for services and determines patient eligibility/suitability for home care services; coordinates with intake for referrals and admissions.
• Oversees scheduling and payor verification/precertification requirements.
• Reviews staff documentation and resolves missing or untimely records.
• Assists clinicians in setting priorities and developing Plan of Care.
• Evaluates patient needs and revises the plan of care in collaboration with clinicians.
• May provide direct patient care in emergencies.
• Participates in the on-call process.
• Assists with staff training to ensure quality care and fiscal responsibility.
• Participates in investigations of patient/family/physician concerns.
