The position emphasizes patient- and family-centered care, on-call availability, and ongoing quality improvement in a compassionate, end-of-life setting.

Qualifications

• RN license in the state of Ohio.
• 1–2 years of acute care experience required.
• Bachelor’s degree in nursing; hospice or home health experience preferred.

Responsibilities

• Provide clinical leadership and oversight for the multidisciplinary team.
• Complete patient and family assessments and develop plans of care.
• Administer medications and treatments per physician orders.
• Educate and counsel patients and families on care plans.
• Ensure compliance with regulatory standards and accreditation surveys.
• Participate in hiring, orientation, and performance evaluations.
• Coordinate care across nursing, therapy, social work, and spiritual care teams.
• Contribute to QAPI initiatives and data analysis.
