Dignity Care Partners of Blue Ash Hospice is seeking a Clinical Manager (RN) to lead clinical operations in the Cincinnati area. This role blends direct nursing practice with multidisciplinary team oversight to deliver compassionate end-of-life care.

You will oversee patient assessments, care planning, staff education, and regulatory readiness, while coordinating care across nursing, therapy, social work, and physicians.

Formación

• Graduate of an accredited school of nursing.
• Active RN license in the state of Ohio.
• One (1) to two (2) years of recent acute care experience required.
• Bachelor’s degree with one (1) year of hospice or home health experience preferred.

Responsabilidades

• Complete initial and ongoing assessments of patients and families.
• Develop individualized plans of care based on terminal prognosis.
• Administer medications and treatments safely per physician orders.
• Provide education, counseling, and health care instructions to patients and families.
• Ensure hospice care and services are delivered according to the plan and regulatory standards.
• Maintain nursing competency and provide relief or direct patient coverage when needed.
• Participate in on-call rotation.
• Oversee care coordination among nursing, therapy, social work, spiritual care, physicians, and caregivers.
• Assist in hiring, orientation, and competency verification of hospice staff.
• Participate in the hospice QAPI program and ensure documentation standards.
