Job summary

Stella Maris Inc. in Timonium, MD is seeking an experienced Hospice Nurse Practitioner to support our Inpatient Hospice Unit and Home Hospice divisions.

You will work under a physician, perform histories and exams, order tests, and develop care plans for hospice patients. The ideal candidate has at least 3 years hospice experience, strong leadership, and excellent teamwork to coordinate with clinical services, nursing staff, and specialists.

Qualifications

• Hospice experience required and leadership abilities.
• Licensed Nurse Practitioner in Maryland with current credentials.
• Minimum 3 years of hospice care experience.

Responsibilities

• Provide medical care to patients in the Inpatient Hospice Unit and Home Hospice division under physician supervision.
• Take medical histories, perform exams, order and interpret tests, and write admission orders.
• Develop and maintain the care plan for medically necessary assessments and services.
• Collaborate with specialists and participate in team conferences and care conferences.
• Explain medical conditions and treatment plans to patients and families; support staff in care planning.
