Job description

Hospice Clinical Liaison (RN/LPN)

Clinical expertise. Trusted relationships. Meaningful growth.

Love patient care but looking for an opportunity to use your nursing experience in a different way?

The Care Team Hospice is seeking a personable, relationship-driven Hospice Clinical Liaison (RN/LPN) to help strengthen our partnerships with hospitals, skilled nursing facilities, assisted living communities, physicians, and other healthcare providers.

This is a unique opportunity for a nurse who enjoys being out in the community, meeting new people, educating healthcare professionals, and helping patients access the care they need.

As a Hospice Clinical Liaison, you'll serve as the connection between our clinical and business development teams, using your nursing background to build credibility with referral partners, identify patients who may benefit from hospice services, and help coordinate a smooth transition into care.

We're looking for someone who is self-driven, outgoing, and comfortable building relationships. The ideal candidate can walk into a facility, connect with nurses and leadership, identify opportunities, and become a trusted clinical resource our referral partners know they can count on.

Whether you have previous hospice liaison experience or you're an experienced RN/LPN ready to explore a new career path, we'd love to have a conversation!

Why Join The Care Team?

At The Care Team Hospice, we're committed to delivering compassionate, patient-centered care through a culture built on Compassion, Innovation, Integrity, and Respect.

We offer a supportive, growth-focused environment along with a comprehensive benefits package for eligible employees, including:

  • Medical, Dental, and Vision Insurance
  • 401(k) with Company Match
  • Paid Time Off and Paid Holidays
  • Tuition Reimbursement and Certification Assistance
  • Mileage Reimbursement
  • Career Growth and Advancement Opportunities
  • Supportive Clinical and Sales Leadership
Key Responsibilities
  • Build and maintain strong relationships with hospitals, skilled nursing facilities, assisted living communities, physicians, and other healthcare referral partners.
  • Serve as a trusted clinical resource by providing education on hospice eligibility, disease progression, symptom management, and the benefits of hospice care.
  • Maintain a consistent presence within assigned facilities, identify patients who may benefit from hospice services, and help facilitate timely referrals and admissions.
  • Partner with clinical leadership, admissions, and business development teams to improve communication, remove barriers, and ensure a positive referral experience.
  • Conduct clinical screenings and eligibility assessments within your professional scope while maintaining accurate and timely documentation.
Qualifications
  • Active and unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of practice.
  • Previous nursing experience required; hospice, home health, palliative care, skilled nursing, or geriatric experience preferred.
  • Strong communication, interpersonal, and relationship-building skills.
  • Comfortable networking, educating healthcare professionals, and developing new relationships.
  • Self-motivated, organized, and confident working independently in the community.
  • Previous clinical liaison, admissions, healthcare sales, or business development experience preferred, but not required.
  • Valid driver's license, reliable transportation, and current automobile insurance required.
