Job summary

The Care Team Hospice is seeking an experienced Hospice Clinical Liaison (RN/LPN) to strengthen partnerships with hospitals, skilled nursing facilities, senior living communities, physicians, and other providers. This role balances clinical expertise with relationship development and education, guiding patients to the right care at the right time.

As a liaison, you will travel within the Midland/Mt. Pleasant corridor, build credibility with nursing leaders, provide hospice education, and support

Qualifications

• Active RN or LPN license in good standing.
• 2+ years hospice or related healthcare experience.
• Knowledge of Medicare Hospice Benefit and eligibility.
• Excellent communication and relationship-building skills.
• Ability to work independently and travel throughout territory.

Responsibilities

• Build and maintain relationships with hospitals, SNFs, and other referral partners.
• Provide hospice education, consultation, and guidance on eligibility and symptom management.
• Maintain presence in assigned facilities to support timely referrals and admissions.
• Collaborate with care liaisons, admissions, and operations to improve referral-to-admission conversion.
• Conduct clinical screenings within RN scope and document referral communications.
