Job summary

OhioHealth at Home in partnership with Compassus is seeking a Hospice Clinical Liaison in Columbus to promote our Caring philosophy and to identify patients whose needs align with hospice services. This role emphasizes collaboration, education, and effective communication with patients, families, and referral sources.

The RN must have 2–3 years of related healthcare experience, prefer nursing degree, and be able to coordinate admissions from hospital or long-term care settings, ensuring smooth

Qualifications

• RN License is required.
• Bachelor's degree in Nursing highly preferred.
• Two to three years of related experience required.
• Hospital and/or long-term care clinical experience highly preferred.

Responsibilities

• Attends staff meetings, educational programs, and community events.
• Explains hospice services and benefits to patients and families to foster understanding.
• Coordinates admissions and ensures all forms are completed and forwarded to Admission Coordinator.
• Establishes professional relationships with referral sources, hospitals, and facilities.
• Facilitates patient discharge from hospital to home and coordinates associated needs.
