Job summary

Hospice of the Western Reserve seeks a Clinical Admissions Liaison-Registered Nurse (RN) based in Ohio. The role involves assessment and admission of patients in home or facility settings, working closely with referrals and the IDT to ensure timely, compassionate hospice care.

Required is a current Ohio RN license and nursing experience in home care or hospice. The position emphasizes HIPAA compliance, documentation, and collaborative patient care delivery within a multidisciplinary team.

Qualifications

• Graduate of an accredited nursing program; BSN and/or CHPN preferred.
• Ohio nursing licensure required; valid driver's license.
• Experience in intake, home care, hospice and/or oncology nursing preferred.

Responsibilities

• Perform information assessment and admission services for referred patients.
• Coordinate with Referral Department and physicians to ensure appropriate referrals.
• Maintain HIPAA compliance and complete documentation in a timely manner.
• Utilize EMR to access and document patient information.
• Collaborate with IDT to coordinate patient care and resources.
