Job summary

Mercy Health Home Care and Hospice by Compassus seeks a Hospice Clinical Liaison to promote Compassus values and identify patients whose care needs align with hospice services. The role emphasizes collaboration with hospitals, facilities, and community partners to facilitate admissions and ensure comprehensive patient support.

The ideal candidate holds an RN license with a nursing degree and 2–3 years of related healthcare experience, including inpatient or long-term care settings, and marketing

Qualifications

• RN license required.
• Bachelor's degree in Nursing highly preferred.
• 2–3 years of related experience required.
• Hospital and/or long-term care clinical experience preferred.
• Experience in healthcare marketing preferred.

Responsibilities

• Attends and participates in staff meetings, educational programs, and community events.
• Communicates identified needs and potential solutions to the Supervisor.
• Explains hospice services and benefits to patients, family members, and significant others.
• Obtains signatures on all required forms and provides copies to patient/responsible party.
• Ensures all forms are completed, dated, signed, and turned in to the Admission Coordinator.
• Ensures all available medical records are turned in to the Admission Coordinator.
• Establishes and maintains significant professional relationships.
• Works with community, hospital, and facility education representatives to meet targets and market acceptance.
• Facilitates admission process to and from hospital and long-term care referral sources.
• Assesses referred patient’s needs and coordinates with physicians regarding eligibility.
